The use of a maternal venous graft for congenital abdominal aortic aneurysm in a newborn: a clinical case

Abstract

Introduction. Congenital abdominal aortic aneurysm in neonates is an extremely rare condition associated with a high risk of rupture and poor outcomes. Limited clinical experience and the absence of standardized treatment strategies necessitate the development of novel surgical approaches. Case description. We present a case of a newborn girl with a prenatally diagnosed infrarenal abdominal aortic aneurysm showing progressive enlargement during the antenatal period. On day 7 of life, due to signs of aneurysm dissection, urgent surgical repair was performed. A maternal venous graft was used for reconstruction. Aortoiliac reconstruction with microsurgical anastomoses was successfully completed. The postoperative course was uneventful. Follow-up ultrasound and computed tomography demonstrated graft patency, laminar blood flow, and signs of vascular wall arterialization. Results. The use of a maternal venous graft enabled successful restoration of aortic blood flow without evidence of thrombosis or stenosis in the early postoperative period. The patient remained hemodynamically stable with a satisfactory clinical condition. Conclusion. Maternal venous grafts may represent a promising option for aortic reconstruction in neonates with congenital abdominal aortic aneurysm, offering potential advantages such as biological compatibility and adaptive growth capacity. Further studies are required to evaluate long-term outcomes.
