The use of prolonged blockade of the spine straightening muscle during endovascular embolization of the spleen in a child with portal hypertension

Abstract

This publication presents a detailed clinical case report of the successful management of a female patient with severe extrahepatic portal hypertension. As part of the interventional treatment, the patient underwent minimally invasive interventional endovascular splenic artery embolization. To optimize perioperative and postoperative pain management within a combined anesthesia regimen, a catheter was placed into the erector spinae plane for prolonged analgesia. The integration of this regional technique into the anesthetic management framework facilitated the principles of multimodal analgesia, reducing intraoperative opioid consumption to the required clinical minimum. In the postoperative period, due to scheduled bolus administration of a local anesthetic, pain was effectively controlled for five consecutive days. Furthermore, the study obtained objective radiographic data that clearly demonstrates the craniocaudal spread of the injected contrast medium within the fascial plane.
